Ela \u00e9 um dos par\u00e2metros de <a href=\"http:\/\/nave.vet.br\/en\/video-aulas\/basico\/monitoracao-anestesica-anestesia-e-o-basico-13\/\" class=\"rank-math-link\">monitora\u00e7\u00e3o<\/a> mais confi\u00e1veis para correlacionarmos \u00e0 profundidade anest\u00e9sica. Al\u00e9m disso, hipotens\u00e3o \u00e9 uma das complica\u00e7\u00f5es mais comuns na anestesia e est\u00e1 envolvida em diversos problemas perioperat\u00f3rios, como insufici\u00eancia renal aguda, hip\u00f3xia do mioc\u00e1rdio e, especialmente em animais de grande porte como equinos, podemos ter problemas na recupera\u00e7\u00e3o, que pode levar at\u00e9 levar ao \u00f3bito do paciente.<\/p>\n\n\n\n<p>Para avaliarmos a onda de press\u00e3o arterial, temos que monitor\u00e1-la de modo invasivo. \u00c9 importante lembrarmos que a mensura\u00e7\u00e3o da press\u00e3o arterial invasiva, por meio de um transdutor de press\u00e3o e monitor multiparam\u00e9trico, nos d\u00e1 uma avalia\u00e7\u00e3o cardiovascular bem melhor do que apenas com um man\u00f4metro aneroide, isso porque conseguimos entender a din\u00e2mica cardiovascular como um todo e n\u00e3o s\u00f3 os valores de press\u00e3o arterial. Para isso, geralmente escolhemos uma art\u00e9ria perif\u00e9rica, que em c\u00e3es e gatos geralmente s\u00e3o as art\u00e9rias podal dorsal ou femoral, em equinos a art\u00e9rias facial, facial transversa ou metatarsal e em ruminantes, coelhos e su\u00ednos a art\u00e9ria auricular.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\" id=\"onda-de-pressao-arterial-padrao\">Onda de Press\u00e3o Arterial Padr\u00e3o<\/h4>\n\n\n\n<p>A onda de Press\u00e3o Arterial Invasiva padr\u00e3o segue o desenho da figura abaixo. Podemos ver um ramo ascendente, em <strong>A<\/strong>, que se inicia com abertura da v\u00e1lvula a\u00f3rtica. O <strong>ponto B<\/strong> representa a Press\u00e3o Arterial Sist\u00f3lica (PAS), ou seja, o pico de press\u00e3o; o <strong>segmento C<\/strong> representa o momento final da s\u00edstole; o <strong>ponto D<\/strong>, que \u00e9 a incisura dicr\u00f3tica, representa o fechamento da v\u00e1lvula a\u00f3rtica; o <strong>segmento E<\/strong> representa a di\u00e1stole e o <strong>ponto F<\/strong>, a Press\u00e3o Arterial Diast\u00f3lica (PAD). Devemos lembrar que a Press\u00e3o Arterial M\u00e9dia (PAM) \u00e9 o produto da (PAS x (2 x PAD)) \/ 3. Onda de Press\u00e3o Arterial padr\u00e3o, com os segmentos relacionados \u00e0 cada momento do fluxo sangu\u00edneo.<\/figcaption><\/figure>\n\n\n\n<p><\/p>\n\n\n\n<hr class=\"wp-block-separator has-css-opacity\"\/>\n\n\n\n<h4 class=\"wp-block-heading\" id=\"alteracoes-na-onda-de-pressao-arterial\">Altera\u00e7\u00f5es na Onda de Press\u00e3o Arterial<\/h4>\n\n\n\n<p>\u00c9 importante sabermos aonde ser\u00e1 feita a arteriopun\u00e7\u00e3o pois h\u00e1 diferen\u00e7as entre as mensura\u00e7\u00f5es. Os vasos de grande calibre, como a aorta, t\u00eam um componente el\u00e1stico muito grande. Por outro lado, as art\u00e9rias perif\u00e9ricas s\u00e3o mais r\u00edgidas, dilatando menos e, com isso, gerando maior resist\u00eancia vascular. Assim, quanto mais longe do cora\u00e7\u00e3o, maior a press\u00e3o arterial sist\u00f3lica. Por outro lado, quanto mais longe do cora\u00e7\u00e3o, menor a press\u00e3o arterial diast\u00f3lica. Apensar dessas diferen\u00e7as, a press\u00e3o arterial m\u00e9dia praticamente n\u00e3o \u00e9 alterada at\u00e9 a 3\u00aa ou 4\u00aa ramifica\u00e7\u00e3o arterial. Padr\u00e3o hipercin\u00e9tico geralmente est\u00e1 relacionado \u00e0 vasodilata\u00e7\u00e3o. O padr\u00e3o hipocin\u00e9tico j\u00e1 est\u00e1 relacionado com vasoconstri\u00e7\u00e3o ou uso de vasopressores. Tamb\u00e9m podemos identificar arritmias, debilidade de v\u00e1lvulas card\u00edacas, aumento de c\u00e2maras card\u00edacas e mesmo altera\u00e7\u00f5es em padr\u00f5es ventilat\u00f3rios. Mas devemos lembrar que o diagn\u00f3stico dessas altera\u00e7\u00f5es deve ser realizado juntando o tra\u00e7ado eletrocardiogr\u00e1fico, os valores de PAS, PAM e PAD, a conforma\u00e7\u00e3o da curva de press\u00e3o arterial, informa\u00e7\u00f5es cl\u00ednicas do paciente e, se poss\u00edvel, outros exames de imagem como ecocardiografia.<\/p>\n\n\n\n<p>Apesar de tentarmos determinar como seriam as ondas de press\u00e3o arterial caracterizadas como normais, n\u00f3s podemos ter algumas interfer\u00eancias que podem comprometer a leitura. Assim, devemos nos atentar em:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Problemas na pun\u00e7\u00e3o: Pun\u00e7\u00f5es sucessivas no mesmo local podem causar hemorragia e co\u00e1gulos, que certamente v\u00e3o diminuir a confiabilidade na avalia\u00e7\u00e3o da press\u00e3o invasiva e tamb\u00e9m da curva de press\u00e3o arterial.<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Calibragem do sistema: A calibragem \u00e9 muito importante. Temos que lembrar que o transdutor deve ficar na altura do cora\u00e7\u00e3o e que devemos zerar o sistema com a press\u00e3o ambiente. Se n\u00e3o fizermos isso no in\u00edcio do processo certamente teremos informa\u00e7\u00f5es erradas.<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Damping: Damping \u00e9 a capacidade de amortecimento do sistema de press\u00e3o. Se o sistema estiver superamortecido ou subamortecido certamente teremos altera\u00e7\u00e3o na onda de press\u00e3o arterial, gerando d\u00favidas na avalia\u00e7\u00e3o. Assim, temos que fazer o teste de lavagem do sistema, verificando a resposta obtida ap\u00f3s isso. Isso geralmente acontece com linhas de press\u00e3o muito longas.<\/figcaption><\/figure>\n\n\n\n<hr class=\"wp-block-separator has-css-opacity\"\/>\n\n\n\n<p>Diante disso, precisamos destacar que a Press\u00e3o Arterial Invasiva nos d\u00e1 um padr\u00e3o de monitora\u00e7\u00e3o bem melhor do que a monitora\u00e7\u00e3o n\u00e3o invasiva, e quando temos acesso \u00e0 onda de press\u00e3o arterial conseguimos entender toda a din\u00e2mica cardiovascular e n\u00e3o s\u00f3 valores de press\u00e3o.
